Where can I order an embroidered chef's jacket?

My nephew is graduating from culinary school in a couple weeks and my husband and I want to get him a chef's jacket with his name embroidered on it. Should I get a specific style and/or fabric? Is there a good place to get one?

One of them being a chef's jacket. So take a look on bravo.com & see if you can do with the one they offer.
